What Are Personality Disorders?

Personality disorders are mental health conditions that involve long-standing patterns of thinking, feeling, behaving and relating to others, creating distress or interfering with daily life. The patterns tend to be more persistent and inflexible than temporary stress reactions.

Depending on the diagnosis and the individual, personality disorders can affect:

  • Emotional regulation
  • Identity and self-image
  • Relationships
  • Trust
  • Impulse control
  • Responses to rejection or criticism
  • Communication
  • Decision-making
  • Coping with stress

Personality disorders involve several diagnoses, including borderline personality disorder, avoidant personality disorder, obsessive-compulsive personality disorder, dependent personality disorder, paranoid personality disorder and narcissistic personality disorder.

Symptoms can look a lot different from one person to another and can overlap with trauma, anxiety, depression, bipolar disorder, attention-deficit/hyperactivity disorder and other mental health conditions. In these situations, a professional assessment is really important because just because someone has certain personality traits, it doesn’t automatically mean they have a personality disorder.

When Personality Patterns Start Interfering with Daily Life

Treatment can be appropriate when recurring emotional, behavioral or relationship patterns cause significant distress or make it hard to maintain stability.

Possible signs include:

  • Intense or rapidly changing emotions
  • Persistent fear of rejection or abandonment
  • Difficulty trusting other people
  • Frequent relationship conflict
  • Impulsive or risky decisions
  • Repeated emotional crises
  • A rigid need for control
  • Avoidance of social situations
  • Difficulty maintaining a stable sense of identity
  • Strong reactions to criticism
  • Repeating harmful relationship patterns
  • Difficulty functioning at work, school, or home

One or two traits don’t confirm a personality disorder, and diagnosis depends on the broader pattern, how long it’s been present, how consistently it appears across situations, and how seriously it affects someone’s life.

How Our Personality Disorder Treatment Program Works

Treatment starts with understanding a client’s emotional patterns, relationships, coping responses, history and goals with the purpose of developing a plan to address the specific areas causing distress or interfering with daily functioning.

Individual Treatment Planning

Our clinical team may consider current symptoms and diagnosis, emotional triggers, relationship patterns, trauma history, impulse control and self-image and identity concerns. Also, as part of individual treatment planning, we’ll look at work, school and family responsibilities, previous treatment experiences, existing coping strategies, co-occurring mental health symptoms, and current safety and stability.

As a client starts building insight, practicing new skills and identifying additional goals, treatment plans can change. Progress isn’t always linear, especially when treatment involves patterns that have developed over many years.

Structured Outpatient Support

Outpatient treatment lets clients receive regular professional care without living at a treatment facility, and it’s a setting that may be a fit for adults who can stay safe in the community but who need more structure than what’s available with occasional therapy sessions. Del Rae Behavioral Health currently offers daytime programming in the morning and afternoon.

Our schedules may help clients consistently dedicate time to treatment while they stay connected to their other responsibilities.

Therapies Used in Personality Disorder Treatment

Therapy can help clients recognize recurring patterns, manage strong emotions, tolerate distress, communicate more effectively and respond differently to conflict, rejection or criticism.

Dialectical Behavior Therapy

Dialectical behavior therapy can be especially relevant if treatment goals include emotional regulation, distress tolerance, mindfulness and healthier relationships. DBT can help with managing intense emotions, responding to distress without acting impulsively, improving communication and setting and maintaining healthier boundaries. It can also help with navigating conflict more effectively, increasing self-awareness and replacing harmful coping patterns with safer responses.

The goal isn’t eliminating difficult emotions, but instead a person can learn how to experience them without letting them control every decision or interaction.

Cognitive behavioral therapy helps look at connections between thoughts, emotions and behaviors, and it may help identify rigid beliefs held about oneself, other people, trust relationships or rejection.

Clients can then start testing those beliefs and practicing more constructive responses. The process can be useful if long-standing assumptions contribute to avoidance, conflict, shame or emotional distress.

Acceptance and commitment therapy helps clients develop more psychological flexibility, so instead of trying to eliminate every uncomfortable thought or feeling they learn how to notice internal experiences and choose actions aligning with their values. The approach may support a more stable sense of direction, especially when strong emotions or negative self-beliefs make it hard to pursue meaningful goals.

When it’s clinically appropriate, treatment may also include eye movement desensitization and reprocessing when trauma contributes to current symptoms, brain mapping, sleep studies and sleep-focused support, and transcranial magnetic stimulation for appropriate co-occurring symptoms.

Not every service is right for every client, so our treatment team will determine which options fit a person’s diagnosis, symptoms, history and safety needs.

Building Healthier Relationships and Daily Stability

Personality disorder treatment often focuses on both internal stability and the ability to manage relationships more effectively. Treatment goals can include recognizing emotional and interpersonal triggers, improving distress tolerance and communicating needs more clearly. Other goals include reducing impulsive reactions, building a more stable sense of self, responding differently to rejection or criticism, replacing avoidance with more effective coping, establishing more consistent routines and planning for continued support after treatment.

Del Rae Behavioral Health also offers holistic services that may complement clinical therapy, including yoga, sound healing, gym or Pilates outings, personal training and sleep-focused services.

These activities don’t replace psychotherapy, but they may support stress management, body awareness, routine and participation in healthier forms of movement.

Alumni and aftercare support can also help clients maintain connection and keep applying the skills they developed during treatment.

Relationship-Centered Care at Del Rae Behavioral Health

Trust and consistency are especially important if treatment involves fear of judgment, struggles trusting others or long-standing relationship patterns.

At Del Rae Behavioral Health, we focus on building genuine relationships with clients and creating an environment where they can talk honestly about emotions, behaviors and experiences. We recognize progress often depends on whether a client feels respected enough to participate openly in treatment.

Our program is co-ed and inclusive, and when possible, clients can also express a preference for working with a therapist who’s a man or woman.

The approach we use moves away from impersonal, standardized care because two people with the same diagnosis may have very different histories, symptoms, responsibilities and goals and their treatment plans should reflect those differences.

Who Can Benefit From Outpatient Personality Disorder Treatment?

Outpatient treatment may be appropriate for adults who:

  • Need more structure than occasional therapy
  • Experience recurring emotional or interpersonal crises
  • Struggle with emotional regulation or impulsivity
  • Want help improving relationships
  • Repeat patterns creating distress or instability
  • Need support to develop boundaries and coping skills
  • Can live safely outside an inpatient or residential setting
  • Want to stay connected to home, work, school or family responsibilities

A higher level of support may be needed during severe crises or when someone can’t be treated safely in the community.

Frequently Asked Questions

Can personality disorders improve with treatment?

Treatment may include dialectical behavior therapy, cognitive behavioral therapy, acceptance and commitment therapy, trauma-informed care and other approaches. The specific combination depends on a client’s symptoms and treatment goals.

There isn’t a fixed timeline. Treatment length depends on symptom severity, established patterns, progress, safety needs and the level of ongoing support needed.

No. Outpatient care can be appropriate for clients who can stay safe in the community. Emergency evaluation or a higher level of care may be needed during severe crises, psychosis or immediate self-harm risk.
